With this enthusiastic support of our Organizing Committee and Editorial Board Member, we are overwhelmed to announce 16th Global Diabetes Conference & Medicare Expo (Diabetes Global 2017) as a continuum of our previous successful conference. Diabetes Global 2017 will be held in Rome, Italy during March 22-23, 2017. The conference will focus on Applying Science and Technology to Fight Diabetes.

The program organizing committee consists of members from all three societies and endeavors to present you with the latest and most exciting data from these respective fields with a range of international and national leading speakers. Track 1: Physiology of diabetes

Pathophysiologic modification is a change in task as notable from a structural defect. Diabetes occurs when there is a dis-balance between the demand and manufacture of the hormone insulin. Pathophysiology of diabetes is that in this circumstance the immune system outbreaks and terminates the beta cells of the pancreas which produce insulin. There is lack of beta cell leading to complete insulin deficiency. Whereas in type 2 diabetes there is comparatively lesser deficiency of insulin and not a complete deficiency. This means that the body is incapable to harvest appropriate insulin to overcome the demand. There is Beta cell deficiency coupled with peripheral insulin resistance. Gestational diabetes is the third type of diabetes. It is caused when there are excessive counter-insulin hormones of pregnancy. This paves the way to a state of insulin resistance and high blood sugar in the mother. Track 2: Diagnosis of Diabetes

Track 15: Diabetic Foot and Ankle

Foot problems are a big risk. All people with diabetes should monitor their feet. If you don't, the consequences can be severe, including amputation. Minor injuries become major emergencies before you know it. With a diabetic foot, a wound as small as a blister from wearing a shoe that's too tight can cause a lot of damage. Diabetes may also decrease your blood flow, so your injuries can be slow to heal. If your wound is not healing, it's at risk for infection. As a diabetic, your infection may spread quickly, and if you have any loss of sensation (neuropathy) you may not recognize that the problem is getting worse. People whose diabetes is not properly controlled tend to have more foot problems than nondiabetics. Having diabetes is not a sentence to developing problems with the feet. Lifestyle and physical activity plays an important role. The patient’s profession can be an indicator to determine what their prognosis may be. Types of diabetic foot and ankle problems seen are multifactorial. The most common complication is peripheral neuropathy. It is often because patients with long-standing diabetes, who have diabetic peripheral neuropathy, may not have adequate feeling in one or both feet or ankles, to know early that something is wrong.

Being overweight increases the chances of developing the common type of diabetes, type 2 diabetes. In this disease, the body makes enough insulin but the cells in the body have become resistant to the salutary action of insulin. Science proposes that being overweight stresses the insides of individual cells. Specifically, overeating stresses the membranous network inside of cells called endoplasmic reticulum (ER). Most patients with type 2 diabetes are obese, and the global epidemic of obesity largely explains the dramatic increase in the incidence and prevalence of type 2 diabetes over the past 20 years. Currently, over a third (34%) of U.S. adults are obese (defined as BMI >30 kg/m2), and over 11% of people aged ≥20 years have diabetes, a prevalence projected to increase to 21% by 2050. Reducing your body weight, by even a small amount, can help improve your body's insulin sensitivity and lower your risk of developing cardiovascular and metabolic conditions such as type 2 diabetes, heart disease and types of cancer.

Track 19: Insulin Management

Insulin therapy is recommended for patients with type 2 diabetes mellitus and an initial A1C level greater than 9 percent, or if diabetes is uncontrolled despite optimal oral glycemic therapy. Insulin therapy may be initiated as augmentation, starting at 0.3 unit per kg, or as replacement, starting at 0.6 to 1.0 unit per kg. Glucose control, adverse effects, cost, adherence, and quality of life need to be considered when choosing therapy. Metformin should be continued if possible because it is proven to reduce all-cause mortality and cardiovascular events in overweight patients with diabetes. Titration of insulin over time is critical to improving glycemic control and preventing diabetes-related complications.

Track 21: Diabetes and Surgery

There is a rising incidence and prevalence of diabetes mellitus. About 50% of people with diabetes mellitus are unaware of their condition. Approximately 25% of all patients with diabetes undergoing surgery are undiagnosed on admission to hospital. Patients with diabetes have a higher risk of cardiovascular disease. Patients with diabetes have a higher perioperative risk. They are more likely because of their disease to require surgery and those undergoing surgery are likely to be less well controlled and to have complications from their diabetes. Surgeons and anaesthetists operating on patients with diabetes should be familiar with the risks attached to having diabetes, and to the particular risks of the particular surgery and of anaesthesia in patients with diabetes.

The term cardiovascular disease (CVD) includes heart disease, stroke and all other diseases of the heart and circulation. Diabetic retinopathy is caused by changes in the blood vessels of the retina. When these blood vessels are damaged, they may leak blood and grow fragile new vessels. When the nerve cells are damaged, vision is impaired. These changes can result in blurring of your vision, hemorrhage into your eye, or, if untreated, retinal detachment. Diabetic retinopathy is the most common diabetic eye disease and a leading cause of blindness. People with untreated diabetes are 25 times more at risk for blindness than the general population. The longer a person has had diabetes, the higher the risk of developing diabetic retinopathy. Fortunately, with regular, proper primary eye care and treatment when necessary, the incidence of severe vision loss has been greatly reduced.

Track 24: Diabetic Nephropathy

Diabetic nephropathy is damage to your kidneys caused by diabetes. In severe cases it can lead to kidney failure. The kidneys have many tiny blood vessels that filter waste from your blood. High blood sugar from diabetes can destroy these blood vessels. Over time, the kidney isn't able to do its job as well. Later it may stop working completely. This is called kidney failure. There are no symptoms in the early stages. So it's important to have regular urine tests to find kidney damage early. Sometimes early kidney damage can be reversed. The problem is diagnosed using simple tests that check for a protein called albumin in the urine.

Importance & Scope:

The prevalence of diabetes for all age-groups worldwide was estimated to be 2.8% in 2000 and 4.4% in 2030. The total number of people with diabetes is projected to rise from 171 million in 2000 to 366 million in 2030. The prevalence of diabetes is higher in men than women, but there are more women with diabetes than men. The urban population in developing countries is projected to double between 2000 and 2030. The most important demographic change to diabetes prevalence across the world appears to be the increase in the proportion of people >65 years of age. These findings indicate that the “diabetes epidemic” will continue even if levels of obesity remain constant. Given the increasing prevalence of obesity, it is likely that these figures provide an underestimate of future diabetes prevalence.

The number of people with diabetes has risen from 108 million in 1980 to 422 million in 2014. The global prevalence of diabetes* among adults over 18 years of age has risen from 4.7% in 1980 to 8.5% in 2014. In 2012, an estimated 1.5 million deaths were directly caused by diabetes and another 2.2 million deaths were attributable to high blood glucose. Almost half of all deaths attributable to high blood glucose occur before the age of 70 years1. WHO projects that diabetes will be the 7th leading cause of death in 2030.

Why Italy?

The prevalence of diabetes as well as glucose intolerance is greater in men and in the regions of the Center and South of Italy. The prevalence of diabetes has doubled in Italy over the past decade and now affects about 3 million people. In Italy, the prevalence of diabetes is 6.6% among people over 20 years old, The Italian Ministry of Health estimates that the number of total undiagnosed cases of diabetes is approximately 2 million. By 2013, the number of cases of both type 1 and type 2 are expected to rise to about 2.7 million and 4 million, respectively. Current diabetes incidence ranges from 4 to 10 new cases per 100,000 inhabitants annually throughout most of the country, but is as high as 35 new cases per 100,000 in Sardinia. Approximately 15,000 new cases of type 1 diabetes and 185,000 new cases of type 2 diabetes are diagnosed nationwide each year, resulting in a and a type 1:type 2 ratio of 1:9. One recent study by the Associazione Medici Diabetologi (AMD, the Italian Association of Diabetes Doctors) found that the number of cases of both type 1 and type 2 diabetes is increasing at about 1% and 7-10% per year, likely due in part to improved screening measures and an increase in obesity and sedentary lifestyles. Reflecting this imperative Diabetes Global-2017 aims to educate pediatricians and achieve the goal of reducing infant mortality rate.

Market Value on Diabetic Research:

The impact of diabetes on the individual quality of life and general economy in Italy is substantial. Healthcare costs for patients with diabetes in Italy are estimated at approximately €5.5 billion annually, representing more than 6% of total national healthcare spending. The Cost of Diabetes in Europe—Type 2 Study(CODE-2), which measured the total healthcare costs for patients with type 2 diabetes in 8 European countries (Italy, Belgium, France, Germany, Netherlands, Spain, Sweden, and UK), found the total yearly (1998) costs for people with type 2 diabetes in Italy alone to be almost €5.2 billion, a per-patient expenditure of at least €2900. Among patients with type 2 diabetes in Italy examined by the CODE-2 study, 17.8% were hospitalized during the 6 month study, with an average hospital stay of 13 days. The cost of hospitalizations accounted for the greatest portion of total costs for all countries in the study, at 55%, leading non-antidiabetic drugs (21%), outpatient care (18%) and antidiabetic drugs (7%).[3] Among non-antidiabetic drugs, the majority were cardiovascular and lipid-lowering (42%).[3] Annual per-patient costs vary widely with primary treatment type, with the highest per-patient costs accrued by those treated with diet and exercise alone.
